Position Summary:

We are seeking a Behavioral Health Clinician (LMSW, LCSW, LMHC, LMFT, Ph.D/Psy.D) to launch our new TEAM First program within our pediatric behavioral health division at Urban Health Plan. 

  • Bilingual English/Spanish is required.

Under the direction of the Behavioral Health Manager, the Behavioral Health Clinician (BHC) provides assessment, consultation, diagnosis, and evidence-based behavioral health interventions for children ages 0-12 and their families. The BHC manages a caseload of patients with behavioral and mental health needs, delivering brief therapeutic services within the primary care setting and coordinating referrals for patients requiring specialty care. Working collaboratively with primary care providers, Community Health Workers, and other team members, the BHC supports whole-person care through consultation, treatment planning, patient education, and care coordination. This role promotes improved health outcomes by utilizing culturally responsive, trauma-informed, and evidence-based practices that address the interaction of physical, behavioral, and social factors affecting health.

 

Minimum Qualifications:

  • Master's Degree in Social Work/related with active NYS Licensure as LMSW, LMHC, LCSW, or LMFT
  • Bilingual English/Spanish is required.
  • Minimum of 2 years of experience providing psychotherapy/clinical social work expertise with pediatric population from birth to 12 and family support services.
  • Early Intervention/Early Childhood background is a huge plus. 

Key Duties:

  • Manage an assigned caseload of patients, providing behavioral health assessment, consultation, brief intervention, and follow-up services for children ages 0-12 within the primary care setting. 
  • Conduct timely, evidence-based assessments to identify behavioral health concerns, confirm diagnoses, assess risk, and develop appropriate treatment recommendations. 
  • Provide brief, evidence-based therapeutic interventions that improve patient functioning and support achievement of treatment goals and document a BH specific care plan in the patients' electronic medical record. 
  • Identify patients whose needs exceed the scope of integrated primary care behavioral health services and facilitate referrals to specialty behavioral health providers when appropriate.
  • Collaborate with primary care providers, Community Health Workers, and other care team members to develop, implement, and monitor patient-centered treatment plans. 
  • Provide consultation and behavioral health expertise to the integrated care team to enhance the identification, management, and prevention of behavioral health concerns. 
  • Educate patients, families, and staff on behavioral health conditions, treatment options, prevention strategies, chronic disease self-management, and wellness promotion. 
  • Utilize validated screening and assessment tools to identify behavioral health needs, monitor patient progress, and support population health initiatives. 
  • Coordinate care with specialty providers, schools, community agencies, and other service organizations to ensure continuity of care and access to appropriate resources. 
  • Maintain accurate, timely, and compliant documentation of assessments, interventions, treatment plans, and patient progress in the electronic health record.  
  • Use culturally responsive, trauma-informed, and family-centered approaches to engage patients and families and incorporate their values, beliefs, and preferences into treatment planning. 
  • Participate in interdisciplinary team meetings, case reviews, quality improvement activities, and program development efforts to strengthen integrated behavioral health services and improve patient outcomes.

Starting Compensation (Based on education, licensure, and experience): 

Minimum $75,000 per year; Maximum $110,000 per year. (note that the top half of salary scale is reserved for advanced LCSW and Psychologist clinicians; LMSW/LMHC salary range is $75,000-$90,000 contingent upon years of clinical experience).
